Founded in 1969, as the South Bay Free Clinic, a small store-front in Manhattan Beach, expanded services in 1980 to the city of Gardena, opening the Gardena Clinic. Our third clinic, the Redondo Beach Clinic, a Patient-Centered Medical Home (PCMH), was opened in 1999; during that same year, on the campus of Carson High School, the Carole Keen/Carson Wellness Center was opened. Subsequently, in 2003, the Dr. Claudia Hampton Clinic in Inglewood opened, and in July 2007, our OB/GYN Satellite Clinic was acquired, also located in Inglewood, an official CenteringPregnancy™ and CenteringParenting™ site. This achievement makes South Bay Family Health Care the only CenteringParenting™ site and one of three CenteringPregnancy™ sites in Los Angeles County. Through our strategically located clinics, South Bay Family Health Care is forging a footprint in access to health care for underserved low-income communities throughout the South Bay and Harbor Gateway.
Our Overarching Goal
To ensure that access to health care is accessible through seven key focus areas, aimed at alleviating social determinants of health by improving health outcomes for the low-income, underserved individuals we serve. Access to Health (primary medical care, chronic disease management, and dental care); Prevention (health and nutrition education, community outreach, community health screenings, and immunizations); Healthy Beginnings (prenatal care, CenteringPregnancy™, CenteringParenting™, and pediatric care); Reproductive Health (full range of birth control options, education regarding sexuality, pregnancy, and STD’s); Adolescent Health (school-based health center, teen clinic, and classroom health education); Mental Health & Wellness (integrated behavioral health and substance use prevention and treatment); Care Coordination/Ancillary and Enabling Services (emergency room discharge coordination, integrated model of care for chronically-ill patients, on-site dispensary, and clinical laboratories).
“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated health care focused on patients”, said NCQA President Margaret E. O’Kane. “Recognition shows that South Bay Family Health Care has the tools, systems, and resources to provide its patients with the right care, at the right time.”
